Position: Case Manager (SAMHSA)
Location: Conroe, TX

Salary: $40,000 annually

HERE'S WHAT YOU'LL BE DOING:

SAMHSA Case Manager is responsible for the provision of case management services to clients of the Homeless Substance Abuse Program, using his/her own personal experience to better assist participants with recovery goals and to develop community support networks. The Case Manager ensures that all requirements of the contract, licensure and VOA-TX Policies and Procedures are applied equally to all clients.

JOB REQUIREMENTS

  • Case Manager will provide and delivery case management to clients, utilizing assessments and Evidence Base Practices.
  • Provides monitoring services for client adherence to Individual Living Plans.
  • Case Manager will assist each participant to navigate healthcare systems and resources needed to achieve and retain sobriety and long term housing.
  • Maintains and strengthens existing linkages with community agencies and establishes new links.
  • Assists residents with coping and problem solving skills.
  • Assists residents in setting appointments and initiating contacts.
  • Provides transportation for residents in facility vehicle.
  • Maintain records and participant files according to funding and agency standards.
  • Case Manager will lead outreach activities to recruit and engage participant's as well as retention strategies.
  • Attends staff meetings and all other required meetings and trainings as directed.
  • Completes all reports required by agency and funding sources.
  • Monitors contracts, ongoing adherence to contract, and administrative documentation for all aspects of program participants.
  • Maintains chronological written records.
QUALIFICATIONS
  • Requires a bachelor's degree plus two years' experience in working with behavioral health services, particularly with homeless populations.
  • This position strongly prefers experience that can offer Peer Support with lived experience in homelessness or substance abuse recovery.
  • Valid Texas Drivers' License.
