About the Opportunity
This position supports the Specialized Family Care Program by providing families, organizations, and communities with technical, resource, and referral support for services of the individuals with developmental and/or intellectual disabilities being placed in Program provider homes. The incumbent will also provide regional recruitment, certification, and training to Specialized Care and Respite Care Providers for approved at-risk individuals.
This position provides program, as well as direct internal and external support services, with the goal of home availability for persons with intellectual and/or developmental disabilities. Responds to inquiries and independently resolves matters of low to moderate complexity. Responsibilities require some judgment, resourcefulness, and originality, but diverse guides and precedents are usually available to apply or adapt to the circumstances. Work effort has considerable impact on client services, program and project image, with potential legal liabilities.

What you’ll do:
- Provides quality assessment support for regulatory, contract, policy and/or program activities and matters.
- Assesses and evaluates providers and facilities for placement of developmentally disabled individuals for long-term and respite care.
- Provides direct services in the field by recruiting, evaluating, maintaining, and monitoring provider- type homes in community settings for I/DD clients.
- Works and interfaces with providers, various representatives in state and local agencies, and consumers.
- Reviews and inspects provider homes and consumer training programs ensuring that they are tailored to the special needs of each client.
- Responsible for oversight of homes providing care for 24 – 30 persons with disabilities, within the assigned region.
- Maintains comprehensive and detailed records including on-going contact and monitoring of the Specialized Family Care Home through monthly visits, quarterly reports, and annual home recertification.
- Designs strategies to attain objectives, informational workbooks and manuals, and provides training to others on these matters.
- Oversees and conducts inquiries. Performs audits and serves as agency audit host.
- Guides internal and external clients on regulatory and program options.
- Performs or assists in program or product development and research.
- Administers and interprets assessment instruments.
- Complies with existing state laws, regulations and guidelines for Medley Case Management.
- Conducts required pre-service training/orientation for Family Care Providers, to prepare them with skills needed to meet the unique needs of each client.
- Trains new Family Based Care Specialists and associated Medley Team members and provides direct input into the development and modification of Bureau of Children and Families policies and procedures impacting Specialized Family Care.
- Plans and participates in regional and statewide meetings on a variety of programmatic issues impacting the Family Based Care Program.
- Participates as a member of interdisciplinary team to collaborate with the Community Case Manager in securing placement while independently outlining total service needs for Medley Class Member or at- risk individuals. These are tailored to the individual needs of each client.
- Attends and participates in institutional and community IDTs/IPPs/MDTs/IEPs for those individuals living in or being referred to Specialized Family Care.
- Bachelor's degree in human services or related field.
- Five (5) years of experience working with I/DD populations, and general knowledge of I/DD issues; experience and skill in working as an interagency team member in planning community placement;
- Or an equivalent combination of education and experience.
- Strong communications skills with the ability to carry out oral and written instructions and multi-task. Must be able to relay information well to others verbally and written in various forms including creating reports.
- Advanced knowledge of processes and procedures, the service and/or product and its application.
- Knowledge of available services, state policy and program limitations.
- Ability to read, analyze and interpret Program policies and procedures, general business periodicals, professional journals, technical procedures or, governmental regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
- Ability to work and meet established deadlines with the ability to effectively communicate orally and narratively with consumers, professionals, providers and co-workers.
- Ability to apply concepts of basic algebra and geometry.
- Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages.
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
- Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
- A knowledge of Specialized Family Care, Foster Care and Adult Family Care programs and all related I/DD services.
- Incumbent will be required to work and respond to a variety of different and unique situations independently using a variety of criteria with a minimum of supervision.
