Job Title:Home Health Registered Nurse

Duties & Responsibilities: RN sought for 13 week assignment with the potential to extend. Schedule if 5x8hr shifts M-F with every 5th weekend on call. 32 hours are guaranteed per week, but 40 are available. The travel area is Cherokee and Forsyth counties. Primary functions of the Registered Nurse are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency and assume the responsibility for coordination of care.

Minimum Requirements:

• 1 year Home Health Care experience
• 1 year HCHB experience
• 1 year OASIS experience
• GA or Compact RN license
• BLS
• Current driver's license and reliable transportation with insurance
• 1-2 years experience as a RN in a clinical care setting
• CHPN certification - preferred
