Salary: $100,000.00 USD Annually - $130,000.00 USD Annually
Description: Our client is currently seeking a Licensed Medical Social Worker

• Education Masters degree in social work.
• Licensed Social Worker (Hawaii) OR Licensed Clinical Social Worker (Hawaii).
• Basic Life Support required at hire from American Heart Association.
• Healthcare experience working with medical diagnoses.
• Demonstrated ability in networking with community agencies.

nice-to-have skills

• one (1) year social work experience in hospital, medical setting, home health, etc. Experience with advanced care directives. Experience in insurance referrals.
• Describe how this position fits in your organization.
• Provides social work services including assessment, care planning, and case coordination to patients and their families. Provides consultation to staff and other health care providers. Initiates social work plan of care. Serves as liason and community to integrate services, coordinate care for clients, and enhance relationships through public relations activities.
