GENERAL SUMMARY/ OVERVIEW STATEMENT:

Under the supervision of the Division Chief, Director of Nursing and Collaborating Physician, the Nurse Practitioner (NP) will work in the Memory Division of the Neurology Department; performing clinic visits (in person and televisits), procedures (lumbar taps), and working closely with other departments and divisions leading clinical continuity. The NP will take a leading role in managing complex patients referred for assessment and care. The NP will help plan, refine, and extend care activities beyond the walls of the clinic to more fully integrate care and improve patients' experience.

The position requires superb organizational skills, interpersonal skills, initiative and innovation. Must be flexible, independent, and willing to participate in multiple activities while providing support to patients by working with the team. Strong communication skills and the ability to converse comfortably with patients and their caregivers are necessary.

PRINCIPAL DUTIES AND RESPONSIBILITIES: Indicate key areas of responsibility, major job duties, special projects and key objectives for this position. These items should be evaluated throughout the year and included in the written annual evaluation.

Clinical Responsibilities:

  • Collaborates with the provider(s) to provide targeted healthcare services to patients within clinic population.
  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician(s).
  • Develops and implements disease-specific patient management plans, records progress notes, provides home-going instructions, and assists in provision of continuity of care.
  • Assesses, instructs and counsels patients regarding compliance with treatments.
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
  • Records physical findings, and formulates plan and prognosis, based on patient's condition.
  • Prescribes or recommends medications or other forms of treatment.
  • Maintains accurate and timely notes in the electronic medical record.
  • Assists physician with comprehensive care.
  • Self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency within specialty of Neurology.
  • Performs other duties as assigned such as, among others, fostering patient awareness of and interest in clinical trials, introducing them to the breadth of trials open for their condition, matching them to the right trial based on detailed eligibility criteria, patient clinical data, and patient and caregiver preferences.
  • Orientation will be on site unless otherwise specified
  • Attendance at regular NP and department level meetings are required

Facilitating Continuity of Care: Oversees and collaborates as necessary in the care of clinic patients.
  • Provides clinical visits.
  • Performs procedures (once competency met)
        • Skin biopsy
        • LP
  • Collaborates with outside facilities and clinical staff for certain patients, and also to provide general information about caring for patients to other caregivers including outside hospitals, PCP's, VNA, and private help.
  • Collaborates with the multi-disciplinary team
  • Explore innovative ways to disseminate patient and community education materials to enhance continuity of care.

Other
  • Provide support and assistance with challenging patient/physician situations. With support of MGH Patient Advocacy as needed.
  • Participate as requested in MGH and Partners quality initiatives.
  • Participate in various hospital and system committees as assigned.
  • Participation may be required in other divisions based on patient care needs within the department of neurology
  • Collaboration with MGB entities as needed

  • Critical Thinking/Decision Making: Ability to appropriately evaluate all aspects of a situation and to independently make appropriate and timely decisions.
  • Ability to coordinate among diverse populations.
  • Discretion and Confidentiality: Ability to handle sensitive and confidential matters discreetly and to ensure confidentiality guidelines are maintained by staff.
  • Safety/Compliance: Knowledge in aspects of safety applicable to position; Knowledge of and compliance with institutional, JCAHO, HIPAA, and other regulatory bodies' standards.
